Virginia Triage Tag Orders

Columbia Business Forms, LLC is the supplier of Triage Tags for the State of Virginia. Our tag is compliant with all “Triage Tag Updates” noted in the Virginia OEMS Board’s 02/02/2024 letter. Virginia OEMS regulations require 25 triage tags in each registered EMS vehicle.

  • Packs of 25, rubber-banded in numerical order and enclosed in clear shrink wrap. Minimum order is 1 pack.
  • Every tag has a 24″ looped string with two 12″ ends to attach to the patient.
  • Delivery is included in the price. All orders ship FedEx Ground; allow 2–3 days after you order.
  • Expedited delivery is available for an added charge. Call Mike Pulaski for details.

How to order

  1. Fill in the online order form below.
  2. Click Print Order Sheet to keep a copy for your records, then click Place Order. Your order comes straight to us.
  3. When the shipment is completed, we will mail you an invoice. Our terms are 30 days. If you prefer, you can also request to pay by credit card.

Virginia Triage Tag, front and back

Our Virginia Triage Tag is built for mass-casualty incidents and tracks each patient from the scene to the hospital.

Triage Tag Features

Size
5½″ x 10½″
String
Each tag includes a 24″ looped string, consisting of two 12″ ends, for secure attachment to the patient.
Identification
Pre-numbered with consecutive Code 3 of 9 barcodes and human-readable numbers with a VA prefix.
Peel-off labels
Each tag features five numbered peel-off labels for affixing to documents and medical paperwork.
Packaging
Sold in packs of 25. Each pack is rubber-banded in numerical order and enclosed in a clear shrink wrap.
